Intensive Case Management
In certain extreme situations, a household may be deemed eligible for Intensive Case Management services. These are designed for the household that has been faced with unforeseen circumstances that threaten the safety and well-being of the household. Each household meeting these basic guidelines may be assessed for intensive services once it is determined that they can benefit from the services and become self-sufficient. It must also be determined that no one in the home presents any history that threatens eligibility for related programs such as Section 8.
Each household that is accepted to this program receives close supervision by a case worker who will monitor compliance with the eligibility criteria as well as a detailed goal plan developed in conjunction with the family or individual. As needed and available, assistance may be provided for transitional living arrangements or temporary housing.
